Resident Service Plans/Agreements can be customized in/ desktop Residex, then printed either from Snap Reports or the Reports tab in desktop Residex.

Agreement: Include Start of Care

The Resident Agreement in ResiDex is a document that can easily be customized to meet your needs.  To customize your agreement, you will choose different component pieces to pull information already entered in Residex, such as demographic information, code status, services, etc....  One component  option will display the move in / intake date AND the start of care date.

Agreement: Assessment of Fall Risk

The Resident Agreement/Service plan contains a component "Assessment of Fall Risk".  We will explain where each of these yes/no questions gather their information.

As you "customize your agreement", you may decide to use the component "Assessment of Fall Risk".  It is helpful to know where the source is for each of these items to make sure the response displays as expected.

To accurately use this component you must use both "CliniDex" (assessments) and "Incident tracking".  Please This email address is being protected from spambots. You need JavaScript enabled to view it. for assistance with your training and implementation of these features.

Agreement: Statement of Home Care Services

The Statement of Home Care Services form may be used to meet the requirement that a home care provider must provide to each client a written statement that identifies the services which the provider is authorized to provide, and which services the provider cannot provide under the scope of the provider’s license, as outlined in MN Stat. Sec. 144A.4791, subd. 3.

ResiDex has implemented this information into the "Resident Agreement" feature.
