Behavior Tracking

Behavior Tracking in ResiDex allows you to assess possible behaviors, create a behavior plan, record behaviors and interventions, and summarize the documented information.

Assess Possible Behaviors

  • In the assessment, you can assess a residents' behavioral issues with the "Behavior issues" assessment category.  (If you need help setting this up, please call 866-512-8369 opt. 2.)
  • The evaluation options include:
  1. No behaviors to monitor
  2. Monitor Agitation/Striking Out
  3. Monitor Anxiety
  4. Monitor Orientation Issues
  5. Monitor Property Destruction
  6. Monitor Repetitive Behavior
  7. Monitor Self Injurious Behavior
  8. Monitor Verbal Aggression
  9. Monitor Wandering
  10. Monitor Other Behavior
  • Choose multiple options that are applicable to the resident's needs and type in optional notes to give more detail.
  • If assessment links are set up, you are also able to automatically schedule related services that allow you to create a behavior plan.

Create a Behavior Plan

  • Manually enter behavior monitoring services in the resident service plan or automatically set up services from the assessment as mentioned above.  These services are set up to coincide with the assessment and will appear on provider assignment sheets and the Today screen.
  • Click on Services in the Resident Profile and select "new" in order to choose from a list of behavior monitoring services.
  • Print a Behavior Plan from the Reports tab.  This will give a list of the Behavior Monitoring services that have been selected for a specific resident.

Record Behaviors and Interventions

  • On the Today screen, click on the service that requires monitoring behaviors and recording interventions (ex. Behavior - Anxiety).
  • Record the number of occurrences by selecting a number from the option list.
  • Record up to 4 interventions that were used in that instance, choosing from an option list.  The interventions include SUCCESS or FAIL information.
  • Click on "done" and the behavior has been recorded.

Summarize the Documented Information

  • The "Behavior Report" can be printed from the Reports tab.  This summarizes the information that had been recorded on the Today screen.
  • Select Behavior Report and choose the specific resident and date parameter.
