What are Assessments and how are they used?

What are Assessments?

Assessments, sometimes called "Clinical Assessments," in senior care, are evaluations of a person's health status.Sometimes Assessments are confused with 'Service Plans.' Equating the service plans and assessments wouldn't be the best comparison; Service plans typically document planned services a health care provider will provide to a client, while an Assessment does not document planned services or indicate an agreement of what will be provided.

The Assessment's purpose is to determine the assessed person's current physical and mental condition, and their needs. Results from Assessments are often used when creating the Service Plan.

The Assessment capability in CliniDex was originally designed over the course of a year with near constant feedback from several nurses. Since then, we have continued to improve it to streamline the Assessment Process.
