Paper vs Paperless

Assessments can be managed in a mixed paper/electronic system.  Or you can manage assessments in a completely paperless way.

Choosing your Assessment Process:

Electronic Assessments (Paperless)

If your organization is trying to reduce the use of paper or would like to be as efficient at using your staffs' time as possible, then capturing the Assessment directly into the computer would be the ideal choice. Doing Assessments electronically allows staff to perform assessments without using any paper at all, recording assessment details directly into the program. Since there is no double entry involved in this method, and because CliniDex is designed to minimize the amount of time it takes to conduct an assessment electronically, this method saves your staff a great deal of time.

Paper Based

However, If you do not have portable computers and have to do the Assessment off-site or doing it directly into the computer would be inconvenient, than printing out a paper version of the Assessment first might be the ideal choice. This does have the downside of you having to do the Assessment by hand first, and then afterward having enter all that data into the computer - so Assessments can take longer to do this way.

Paper-Based Assessments

Performing assessments on paper allows you to be more portable, if you do not have the ability to have a computer present when conducting assessments, allowing staff to conduct assessments in person (on paper,) and then record the data into CliniDex at a later time.

If you do decide to do the Assessment on paper, then you may need help figuring out which paper-based form you should print out to give your desired Assessment.

Performing and documenting an assessment will be easier if you use the right form. Each assessment has a blank form, a worksheet form, and a summary. Which one should you use? Choose one of the following scenarios to help guide you through choosing the right form.

Pre-Admission Assessment

If you wish to perform a pre-move-in Assessment for a prospective resident, then you will want to use the "Pre-Admission Assessment (Blank)" from. This form lists all of the questions from the Pre-Admission Assessment, and all possible responses.

Admission Assessment

When a prospect has been admitted to your facility, has become a Resident, and has already had a Pre-Admission Assessment conducted, you will want to conduct an Admission Assessment. In this scenario, you will want to use the "Admission Assessment Worksheet" form. This worksheet will show all of the previously entered (Pre-Admission Assessment) information, while still showing other available responses.

Supervisory Visit, Clinical Update, or Transfer / Discharge

In the event you are doing an Assessment for a Resident who has had an Admission Assessment previously, you will want to use one of the following forms, as appropriate for your scenario:

  • Clinical Update Worksheet
  • Supervisory Visit Worksheet
  • Transfer or Discharge Worksheet

Each of these forms will show all previously entered Assessment Information, while still showing the other available responses.

Finding and Printing your forms

  • To find and print the form you need, go to the Reports tab of ResiDex.
  • In the "Search:" field of the Reports tab, type the exact name of the form you need; this will filter out all of the other report and forms except for the one you need
  • Click the go button next to this form
  • Select the name of the Resident you wish to generate this report for.
  • Preview or print the form as you desire
