How to conduct an Assessment straight from a computer, or on a paper-based Assessment form

Entering Assessment Information
Start from the Profile of the Resident you wish to do an Assessment for.
- Open the “Resident Option List,” and select the Occasion you want to give an Assessment for
- You will be asked if you would like to start the Assessment – Select “OK” or “Yes;” This launches the Assessment Screen
- This Screen is divided into the Assessment Sections of CliniDex. These are listed by their abbreviations at the top of the screen:
- “Ad” – Admission
- “Ind” – Independent Living
- “ADL” – Activities of Daily Life / Activities of Daily Living
- “Me” – Medication
- “Phy” – Physical
- “Soci” – Social & Cognitive
- “Saf” – Safety
Each section has a series of Categories that must be Evaluated to complete the Assessment. These are the “Assessment Categories” (also known as “Assessment Questions”) and “Evaluation Options” that were discussed in the training for CliniDex Setup.
You can work in whatever order of Assessment Sections you wish – just click on the desired Section to go to it’s Categories. You can also use the Arrow keys at the top of the screen to move between Sections. In between the Arrow keys there is statement that says “Remaining: (X) of (Y)” – this indicates there are (X) Categories still needing to be evaluated, out of the total (Y) Categories in this Assessment.
An Assessment can be done all at once, or spread out over a period of time. You can pause an assessment by simply exiting the screen at any time during the Assessment – it will not be considered to be finished until you mark the Assessment complete (more on this later)

To Evaluate a Category:
- Click on the Category on the left side of the screen that you wish to evaluate: this will list that Category’s Evaluation Option(s) on the right side of the screen. Categories can be evaluated by simply selecting one or more of the options on the right side of the screen. To select an Evaluation Option, simply click on it; it will then become highlighted, indicating that is the selected Option.
- Some Evaluation options will require a little more information and, when selected, will prompt you to type in some text to into a description, or select some options from a menu.
- Some Categories have their Evaluation Options grouped into Sets. This means that unless you have an unusual or special circumstance, you would only select one answer from a Set of Evaluation Options. On rare occasions, multiple Options from the same Set would be selected – you will be warned when doing this that it is unusual and generally not recommended.
- Some Evaluation Options will trigger the creation of Services within ResiDex for the Resident being evaluated; we call this Assessment and Service Plan integration.
- Some Evaluation Options indicate that another provider will be providing a service for a resident – if the “Other Providers” menu doesn't have the choices you desire, you can take a look at the Other Provide Types (for Assessments) tutorial, which will show you how to add providers to this list.
- If you feel that your Evaluation for a particular category was not as clear as it should be, or needs some more explanation, you can highlight your selected Evaluation option, which will give you an “Add Note” button at the bottom of the screen. You can press this button to type in more information related to this Category.
- To finish an Assessment, make sure that you have evaluated every Assessment Category, by verifying the top of the screen says “Remaining: 0 out of #,” and then Press the “Mark Complete" Button at the top of the screen
